Special Graphics 
There are two spacer graphics used for the generated HTML:
s.gif
sh.gif
These are both spacers. The s.gif image is 1 pixel wide by 875 tall. The sh.gif image is 545 pixels wide by 1 pixel high.
These graphics are used to contain the text inside the main entry box, yet maintain the size of the other tables on the page. Note that there are better ways to do this as far as website design goes; however, this helps to make the MCJ easier to use.
Just put these in the root directory of your website.
There is also a small graphic for the printer icon. Use the one below or make your own that isn't tractor feed:
